Patient Contact Details & Practice Boundary

Phone numbers & email addresses

We need to ensure that your contact details on your medical record are up to date. This is important so we can reach you promptly when necessary. While we normally contact you by phone, we may use email rarely if we're unable to reach you. Please provide us with your current phone number and email address, and feel free to list more than one of each if needed. At a minimum, we require your current mobile or landline number for your medical records. 

 

Residential address 

Please inform us when you move house so we can update your address on your medical record. This is crucial for several reasons: 

  • If one of our clinicians refers you for further investigations, the address on your file will be included in the referral. This is the address the hospital or secondary care department will use to send your appointment details. If it's incorrect, you may miss important communications about your care.

  • In the event that a home visit is required, having the correct address is vital to ensure timely care.

  • In the case of urgent or time-sensitive information, such as test results or important practice information, we may need to contact you by mail if we are unable to reach you by phone. An updated address helps prevent any important communications from being delayed or lost.

  • We also need to ensure that you are still residing within the practice boundary. If you've moved outside of this area, unfortunately, you will no longer be eligible to remain registered with Maryhill. (See below for more information on the practice boundary).

Practice Boundary 

Our practice boundary covers a large area of Moray, including the main towns of Elgin, Lhanbryde, Lossiemouth, Hopeman, Burghead, Roseisle, Alves, and Rothes, as well as numerous smaller villages and hamlets. A detailed map showing the exact boundary and all covered locations is available on the noticeboard at the front desk. 

Practice boundaries are set to allow GPs and clinical teams to provide care effectively for the population within that area. This includes home visits if you are too unwell or elderly to attend the practice. It can become difficult and unsafe to try to provide home care for patients who live outside the boundary, and for these reasons it is recommended you register at a practice closer to your home address. 

Some towns outside our boundary include Aberlour, Craigellachie, Dufftown, Forres, Kinloss, Fochabers, Mosstodloch, and Garmouth, though this list is not exhaustive. If you live in one of these towns and wish to register with us, we won’t be able to accept your registration.   

Please note that while we are not actively reviewing our current patient list to remove individuals already registered from these towns, we strongly recommend that you register with a practice whose boundary includes your home address. Clinicians may discuss this with you during appointments and will explain the benefits of living closer to your practice.
