Patient Consent

Patient Consent for Others to Discuss Their Health 

This week, we're focusing on patient consent and the importance of protecting confidentiality. If you contact the practice to discuss another patient’s health or request their test results, we must have their written consent. This ensures that we are safeguarding our patients' right to privacy.
 
For patients aged 16 and over, we require direct signed consent from them before sharing any information. If a patient wishes to grant someone else access to their records, they need to contact the practice, request a consent form, and complete it by providing both their signature and the details of the person who will have access.
 
For younger patients approaching the age of 16, consent matters can be more nuanced and are usually discussed with both the child and, where appropriate, their parents or guardians by the clinician. 

How to Set This Up 

To arrange consent for somebody else to have permission to discuss your health, please ask for Consent Form by eConsult, on 0345 337 0610 or at the front desk, where we will guide them through the process of completing the required form. 

Power of Attorney (POA) / Next of Kin (NOK) 

If you are a Power of Attorney (POA) for a patient, but the POA is not yet active, we still need written consent from the patient to share information with you. Similarly, being listed as Next of Kin (NOK) does not automatically grant access to a patient’s records. In both cases, we request that you complete our internal consent form for our records.
 
Additionally, if you are a registered POA for a patient, please provide us with a copy of the documentation so we can update the patient’s notes accordingly.
 
Please note that GPs no longer handle the process of establishing Power of Attorney. If you need to set one up, we recommend contacting your solicitor.
