FLU and COVID-19 booster IMMUNISATIONS

As you are aware the practice will again not be carrying out you flu immunisations this year. Please do not call the practice to make flu appointments

Flu Call Centre –  0345 337 9899,    Email – Gram.flu-info@nhs.scot

In 2017, the Scottish Government and the Scottish General Practitioners Committee (SGPC) agreed vaccinations would move away from a model based on GP delivery to one based on NHS Board delivery through dedicated teams.

There followed the significant work around the COVID-19 vaccination program that is ongoing.

Clinics will again be set up in a number of locations across Grampian within secondary schools and community spaces such as sports halls, churches and any other large venues that can allow for social distancing. Measures will be put in place to ensure health and safety is priority.”

The www.nhsinform.scot website will have information about eligible groups for both Flu and COVID-19 boosters when this information is released.

Online 'Pre-Registration' With The Practice

PreRegistrationIf you wish to register click on the link below to open the form. When you have completed all of the details print the form and sign it. You will still need to visit the surgery to hand in the form. You will have to bring some identification with you (driving licence, passport or any utility bill). If you are on any repeat medications bring a copy of your re-order prescription form from your previous practice. Please complete a separate form for each member of the family/household

STEP 1: GPR form.doc

We also need you to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this form, which  you must fill out and and print off . When you come to the surgery you will then be asked to sign this form to confirm that the details are correct. Again a seperate will need to be completed for each of the family/ household member

Online appointment booking is the most used GP online service. Patients avoid queues at the practice or long waits on the phone. As a result it eases pressure on busy reception desks. It’s especially popular over the weekend so there are fewer calls on Monday mornings.
